Acrobat 9 - allow end users to save form, but not make comments?

i've used acrobat 9 pro create pdf form must allow recipients fill in printing , saving (form fields only). fill in, not save in original format. per suggestions on forum, enabled "extend features in adobe reader..." user can save form after filling out. however, opened ability modify document, add comments, etc. when add security measures prevent this, ability save form entries disappears again.

what have end users can fill out form , print/save changes, not edit or comment on form itself?
